A 25-year-old woman presented with a 6-month history of increasing shortness of breath on effort and several episodes of exertional syncope. On examination, she was not clubbed and there were no cardiac murmurs. Echocardiogram showed normal valvular function and no intra-cardiac mass. The chest X-ray showed clear lung fields.

A 34-year-old woman was referred for the investigation of bloody diarrhoea. She was opening her bowels four times daily. On examination, she was well. Her pulse was 64 beats per minute and her abdomen was soft and non-tender. Colonoscopy revealed an active colitis, limited to the sigmoid colon. Biopsies confirmed a diagnosis of ulcerative colitis.

Investigations:

white cell count 11.5 x 109/L (4.0 - 11.0)

A 28-year-old woman gave a 3-day history of ascending sensory disturbance, beginning in her feet. She had noticed progressive difficulty in climbing stairs, with some low backache. She had difficulty in voiding urine.

On examination, she had normal cranial nerve function. Power was normal in the arms. Her legs showed moderate proximal weakness but more severe distal weakness. She was areflexic and the plantar responses were flexor. Sensory testing was normal.
What is the most likely diagnosis?
